Medicaid partner programs
Celestial Private Homecare, LLC is a proud Medicaid provider for the Elderly & Disabled Waiver Program (EDWP) and participates in the following waiver programs:
SOURCE
Service Options Using Resources in a Community Environment (SOURCE) serves frail, elderly, and disabled Georgians. It provides coordinated services in their home or community. SOURCE can provide adult day care, alternative living services, personal care, home delivered meals, and respite care for family caregivers.
CCSP
Georgia's Community Care Services Program (CCSP), a Medicaid-funded program, provides Home and Community-Based Services (HCBS) for state residents who are elderly and / or disabled, functionally impaired, and at risk of nursing home admittance. CCSP is now part of the Elderly and Disabled Waiver Program.
ICWP
The Independent Care Waiver Program (ICWP) in Georgia helps Medicaid members with physical disabilities or traumatic brain injuries live in their own homes. ICWP is a Medicaid waiver program that operates through the Georgia Department of Community Health (DCH).
We provide high-quality, compassionate care to eligible individuals under these programs, ensuring they receive the support and assistance they need to maintain their independence and well-being.
